A: Studies have demonstrated a lowered risk for HIV transmission among circumcised men. For information, you can read about these studies at the Centers for Disease Control.

Studies about sexual pleasure among circumcised and non-circumsised men are rather variable, incidentally. Some studies find that men report a difference, and some do not.

Q: What’s the difference between sex with a guy who’s cut vs. uncut? My current boyfriend is foreign and will be moving here within two weeks. It’ll be his first time overall — so naturally he’s terrified — and my first time with an uncut guy. Is it going to make a difference in how I feel or how he feels?

A: You will notice very little difference between a cut and uncut (circumcised and uncircumcised) male penis as far as you are concerned. The foreskin consists of the same skin that is on the head of the penis that folds over part or all of the head when the penis is flaccid (soft) or folds onto the side of the penis during erections. There are nerve endings in the foreskin that are also similar to the nerve endings elsewhere on the penis so this skin is sensitive and can be the source of added pleasure for the uncircumcised person.

The main difference has to do with hygiene as it relates to the uncircumcised penis needing more attention than the circumcised penis in cleaning and keeping the area clean. The area of skin around the head of the uncut penis can tend to have a stronger odor at times than the cut penis if the hygiene is lacking.

There are a few other things to know about the uncut penis such as some men prefer to have the foreskin pulled back prior to receiving oral sex and/or putting on a condom. Also, lube can be even more helpful on the uncircumcised penis, either with or without a condom, to reduce the amount of friction between the loose skin and the rest of the penis.

Q: I have tried all kinds of solutions, but every time I have sex with a girl I come within a matter of minutes. Same thing for when I masturbate. I am 19 and I live in a college town where I frequently hook up with girls. Every time I have sex it seems that I lose more and more self esteem. The only times I have ever been able to control my ejaculation/climax is when I took MDMA (ecstasy). I literally went for hours. I would love to be able to do that every time without hurting my body. I have taken 5-HTP because I thought my serotonin levels were low, but still nothing. I feel like my sex life is hopeless. Please help me.

A: First of all, 2-10 minutes is actually the typical time for a male to reach orgasm, and at your age it is absolutely normal to reach orgasm very quickly. Secondly, many girls would prefer a shorter session to a session that lasts for hours because long sessions of PV sex can lead to pain for women.

As far as suggestions go:

-One thing you can do (if you’re comfortable with masturbation) is to bring yourself close to the point of orgasm, then pause to allow the orgasm cycle to relax, then continue until you get close again, and repeat. This can help train yourself into lasting longer.

-You can also take short breaks during intercourse (engage in other sexual activity- such as oral sex) to essentially do the same thing and extend the session.

-They also make condoms that are thicker than usual to decrease sensitivity, and condoms that contain a desensitizing substance on the inside (Durex Performax, for example).

Also, worrying/thinking about coming too soon will be likely to increase the chance of it happening. Try to relax, have a good time, and you’ll be fine.

Q: I am uncircumcised and I had a few questions first, I can’t find a condom that works with my penis. The foreskin moves up and down and and the condom starts to ride up my penis and bunch up and hang off at the end. I was wondering if there was a certain kind of condom for uncircumcised guys so the foreskin could move freely. Second, some times after I have sex my forekin gets real tight and I can’t retract it and if I try to it starts to tear and is really tight. I don’t know what it is from. I have a long overhang and I can always retract my foreskin just fine except once in a while it’ll get tight. Third is it normal for women to bite and pull on the foreskin when giving oral sex? It dosen’t hurt much but this girl always bites and pulls it.

A: It is possible to find a condom that will work for you. Condoms come in hundreds of shapes and sizes. If you are able to browse the internet, please check out this site, www.condomania.com. You don’t have to buy from there, but you can look at the brands and styles and then go to your local store to buy them.

Also check out this page on the site which shows condoms with more headroom and a tight fitting base.
http://secure.condomania.com/products.asp?dept=40

In general, ensure the condom is applied correctly. Pull the foreskin back before putting it on, then push forward over the head of the penis once the condom been fully unrolled.

We are not sure why your foreskin became tight. It could be due to temperature or arousal levels. It sounds like it recovered so that’s good news. For your girlfriend who bites and pulls on your foreskin, you will just need to tell her what you like. She is probably just trying to please you and find out what feels good. The skin may be fun to play with. If you like it you should tell her or make happy noises. If you want her to do something else then tell her that too.

Q: What is the actual size for a 14 year old boy’s penis?

A: Thank you for your question. The length and circumference of the penis of boys will change as they grow older and go through puberty. Puberty is the process of physical changes by which a child’s body becomes an adult body capable of reproduction. Puberty will occur at different ages for different boys, for some it can start as early as 9 and for others not until 14 or even later. Two fourteen years old boys can have penis sizes that are significantly different. Some boys will not have achieve their adult size penis until late in their teen years. Most adult men will have a penis that is about 4 inches long when flaccid and 5 to 7 inches in length when erect.

Q: I am 23 years old and I am considering of getting circumcised. Would you please tell me what this procedure is like? Is it done by the urologist? Does it require full anesthesia? How long should I stay in hospital? How is the recovery?

A: Adult circumcision is usually done on an outpatient basis by a urologist so there is no hospital stay, and is generally done with local anesthesia and you are awake for the procedure.

There are two techniques that are used to perform the circumcision. The dorsal slit technique involves cutting a small slit down part of the length of the foreskin then the foreskin is pulled back and cut away and sutured.

The second technique is called the sleeve technique. A skin marker is used to mark the incisions and the foreskin is cut around the body of the penis so that a “sleeve” of skin is formed. The foreskin is then separated and removed and sutured.

As with any surgical procedure there are potential complications. Your doctor should be able to tell you about these in detail.

Aftercare involves gentle washing of the wound daily for 5-7 days after which you may shower regularly. Masturbation and intercourse should be avoided for 4-6 weeks after the procedure. But, again, your doctor can give you specific details about aftercare.

For a less medically focused information on adult circumcision, you can visit Go Ask Alice.

Q: My 60 year old boyfriend is uncircumcised. When he retracts his foreskin it tears. He has very fair, delicate skin. His foreskin does not retract easily even when his penis is flaccid.

What can a man do to gradually stretch the foreskin so that it does not tear during intercourse?

A: The difficulty with retracting the foreskin is called “phimosis”, and it isn’t uncommon in uncircumcised men. It can have different causes, so it might not be a bad idea to go get looked at by a knowledgeable doctor to narrow things down. Most commonly, doctors will recommend treatment by circumcision or by a steroid cream. Some people also have success with simply manually stretching the foreskin–gently, evenly, and frequently. One tool made to help with the manual approach can be found here: www.glansie.com

Q: If I want to give a guy a handjob how do I go about that? I mean is it just about the penis or the other stuff, too? And how do I do it? What makes it feel good for a guy?

A: First of all, keep in mind that every guy is different, and that what feels good for one guy may not be great for another guy. So, the most important part of handjobs–or any kind of sex, for that matter–is communication with your partner. This could mean just paying attention to the way he responds to what you’re doing as you try different things, or asking him what he likes. He might even be willing to show you how he likes to be touched.

The general idea of a handjob is to wrap a hand around the shaft of a penis and to stroke it up and down, much the same way a guy would masturbate by himself. However, there are many, many variations: dry or with lubrication, varying the grip, speed, or firmness, or stimulating other areas as well. Both men and women have a high concentration of nerve endings in the genital area, which is what makes it feel good to be touched there. On men, these sensitive areas generally include the head and shaft of the penis, the balls, the skin around and underneath the balls, and even the anus (though many guys are not comfortable being touched there). The most sensitive place on a guy’s penis is typically on the underside, where the head meets the shaft, but just like the clitoris, this can sometimes get oversensitive if it is stimulated too much.

If you’d like to know a little more about techniques that guys use on themselves, you can check out this website on male masturbation called Jackinworld.

For more on this and other general sex information, you might want to check out a book called Guide to Getting It On, published by Goofy Foot Press.

I’m very self conscious about my virginity (at 33) and the fact that I have a (pathetic, in my opinion) 4 inch cock. I really want to have sex with this woman. Should I tell her, before, about my virginity and 4 inch cock. Or, should I just try my best?

A: Good communication is often a key to positive sexual experience. However, you don’t have to spell everything out right away if it makes you feel uncomfortable. You can mention that you are less experienced, and would want your partner’s instructions. It might start an honest dialogue, an opportunity for her to explain to you what she likes. Also, relax, sex is not a marathon. Take it slow, explore each others bodies and minds at a speed and comfort level compatible to you. Pay attention to what you like and she likes, and you will probably start learning very fast.

Many men worry about the size of their cock. However, there are plenty of men of all sizes that have satisfying sexual relationships. Your cock size probably won’t be a big issue, but should this be a problem for your partner, there are alternate sexual positions you can try together. On all fours with a partner entering vagina from behind (doggy style) helps to increase the depth of penetration. You can place a pillow under her abdomen for her comfort. Some people also like to use sex toys, and/or mutual masturbation, and/or oral sex. Some women care about full penetration, others don’t, and prefer foreplay, oral stimulation, or other forms of sex play instead. Communicating about it is the only way to know for sure!
